Cyber Incident Form — For Investors

If you received a letter from CIRO or an email from ciro@notifications.cyberscout.com they are legitimate and it is safe to follow the instructions.

If you received notification from CIRO offering you credit monitoring and identity theft protection through TransUnion and Equifax, we encourage you to sign up for these services as instructed in the notification you received.

Not all current or former clients of firms that are CIRO member firms (or were members of CIRO’s predecessor organizations) were impacted by the cybersecurity breach. If you did not receive a notice from CIRO about the incident but contacted us because you are a current or former client of a CIRO member firm and believe you should have received notice, please note that you will only receive a response if CIRO determines that you should have been notified.

Please review the FAQs prior to using this form. The FAQs section covers most commonly asked questions, including:

  • What personal information of mine has been exposed
  • Why CIRO has my information

If you have questions not covered in the FAQ, please proceed with the Cyber Incident form.
